St. Nicholas Day Event Response Form

When: Thursday, December 7, 2023 at 7pm ET / 4pm PT (Please be sure to mark your calendar.)
Where: Zoom link will be sent closer to the event to those who RSVP.

St. Nicholas himself will visit Lotus & Ivy via Zoom and read about each student from his Golden Book. If you plan to have your child attend, please fill out this form, 1 per child with the name of your child and the answers to the following questions. (Children in the household Grade 2 and under may participate.)

ALL FORMS ARE DUE BY NOVEMBER 18, 2023. Late forms cannot be accepted due to many moving parts to make sure St. Nicholas has what he needs for the event.
